Important Notice for Pensioners Attending the Pension Activation Interview!
Essential Documents Required for the Interview
1. Primary Identification Documents
- Your National Identity Card (NIC), a clear photocopy of it, and your original Birth Certificate.
- A photocopy of your spouse’s NIC and their original Birth Certificate.
- The Widows’ and Orphans’ Pension (W&OP) Card.
2. Marriage, Divorce, or Death Certificates (As Applicable)
- The original Marriage Certificate, or the original absolute divorce decree if legally divorced.
- The original Death Certificate if your spouse is deceased.
- Legal documentation proving the lawful dissolution of any previous marriages (if applicable).
3. Certificates Regarding Children
- Original Birth Certificates of any children under the age of 26.
- For disabled children: Medical reports, clinic records, and diagnostic histories proving the disability occurred before reaching age 26. You must also bring a postcard-sized color photograph of the child, the Health 307 report, and their original Birth Certificate.
4. Other Special Documents
- One (1) recent passport-sized color photograph of your spouse.
- Affidavits or supporting legal documents if there are any discrepancies or changes in names across your paperwork.
- A formal letter from the Head of the Department where you last served, certifying that W&OP contributions were properly deducted throughout your entire tenure. (Note: This letter is NOT required for pensioners whose applications were entered into the online system on or after June 12, 2019.)
Special Attention:
* All birth, marriage, and death certificates must be official original copies issued by a Divisional Secretariat. Free promotional or temporary copies will not be accepted.
* All submitted original documents will be verified and returned to you on the same day. If you have not contributed to the W&OP fund, these specific family documents are not required, and only your Pensioner ID card will be processed.
Interview Dates and Timelines
Attendance is strictly mandatory on the exact date and time allocated to you. If an emergency arises and you absolutely must reschedule, you are required to call the official hotline at 1970 immediately.
