Essential Documents Required for the Interview

1. Primary Identification Documents

• Your National Identity Card (NIC), a clear photocopy of it, and your original Birth Certificate.
• A photocopy of your spouse’s NIC and their original Birth Certificate.
• The Widows’ and Orphans’ Pension (W&OP) Card.

2. Marriage, Divorce, or Death Certificates (As Applicable)

• The original Marriage Certificate, or the original absolute divorce decree if legally divorced.
• The original Death Certificate if your spouse is deceased.
• Legal documentation proving the lawful dissolution of any previous marriages (if applicable).

3. Certificates Regarding Children

• Original Birth Certificates of any children under the age of 26.
• For disabled children: Medical reports, clinic records, and diagnostic histories proving the disability occurred before reaching age 26. You must also bring a postcard-sized color photograph of the child, the Health 307 report, and their original Birth Certificate.

4. Other Special Documents

• One (1) recent passport-sized color photograph of your spouse.
• Affidavits or supporting legal documents if there are any discrepancies or changes in names across your paperwork.
• A formal letter from the Head of the Department where you last served, certifying that W&OP contributions were properly deducted throughout your entire tenure. (Note: This letter is NOT required for pensioners whose applications were entered into the online system on or after June 12, 2019.)
